Please, carefully read the NOTICE below and follow the directive there.

STOP SENDING forms to us here as all forms are to be submitted as HARD COPY.
Also, anyone filling the share transfer form must fill the e-dividend form too and submit both together as hard copy to the nearest DPOs/Command MSS Office /Command Coop Office.
Return the HARD COPY of completed e-dividend form to us through your nearest DPOs, Command MSS Offices and Command Cooperative offices.

STEP 1.
PROCEDURE ON HOW TO COMPLETE THE POLICE MORTGAGE BANK E-DIVIDEND FORM:
1. Download the form here

2. Print out the form
3. Carefully fill the form
4. Take it to your commercial Bank that the e-dividend will be paid into (The same bank details imputed in the e-dividend form) for verification and stamping/signing.
5. Return the HARD COPY of completed e-dividend form to us through your nearest DPOs, Command MSS Offices and Command Cooperative offices.
